APP PRIVACY STATEMENT

General This is the privacy statement of the CareXS app. In the privacy statement, you will find important information about the processing of your personal data via this app. We recommend that you read this privacy statement carefully before using the app.

Objective The personal data mentioned below are processed for the following purposes:

  • providing access to your (or your clients’) medication status and medication data;
  • registering medication use, data for medical treatment if you have thrombosis, and informing the relevant healthcare parties about this;
  • sending important notices and announcements, for example, about changes to the terms of use, this privacy policy, and software updates.

Which personal data do we use: The following data are processed in CareXS:

  • General data: name, address, postal code, city, date of birth, BSN number, telephone number, gender, email address.
  • Medication data:
  • medication name
  • date of provision
  • recommended dosage
  • use of the medication
  • if you have thrombosis, your blood values and treatment data are also recorded.
  • Data about the parties involved in the care, including the general practice, pharmacy, specialist, home care institution, and thrombosis service.

Consent

The aforementioned personal data are only processed in CareXS if the client has given explicit consent for this, unless an involved healthcare party reasonably judges that they are legally obliged to provide your data.

Retention period The personal data processed via CareXS are stored for a maximum of five years, calculated from the time they were created, unless the client objects to this.

Security CareXS processes your personal data in accordance with the requirements of the General Data Protection Regulation, the applicable provisions of the Medical Treatment Agreement Act, and the guidelines of the Dutch Data Protection Authority. This means that personal data are protected as well as possible against loss, misuse, unauthorized access, disclosure, modification, or destruction.

Your rights You always have control over your personal data. You have the right to view, correct, or delete your personal data. You can send a request for access, correction, or deletion to info@tsc-cc.com. Connected Care B.V. will respond to your request as soon as possible, but within four weeks.
